The trochlear, oculomotor, and abducens nerves pass through the region, the internal carotid artery, and the ophthalmic vein; because they do not have valves, they allow perinasal infections to migrate to the sinus, causing the septic thrombosis. This case report is about a 22-year-old female patient who used an ala/wing nose piercing during 2 years. After an inflammatory reaction, local edema, fever, pain, discreet projection of the eyeball and exudate draining spontaneously through the piercing orifice was perceived. The tomographic examination and magnetic resonance evidenced the thickening of the cavernous sinus and the presence of secretion. For treatment the piercing was removed, nasal secretion was drained, and oxacillin, gentamicin, and metronidazole were used intravenously with a semi-intensive accompaniment for 7 days.
