- W3087955692 abstract "Food allergies involving PR 10 are known for their clinical benignity (mostly transient oral syndromes). However, we do report 2 unusual cases of hazelnut anaphylaxis that were first confused with a peanut allergy and whose results highlighted the responsibility of PR10. We report 2 observations of grade 3 and 4 anaphylaxis to hazelnut, one in a 13-year-old girl with a known peanut allergy and an important atopic history, the other in an 18-year-old male allergic to Betulaceae pollens. In both cases peanut was suspected because of the 1st patient’s medical history, and because of the 2nd patient’s chronology of symptoms: the latter had just eaten a peanut-based candy. Peanut was exonerated for both patients: the girl had not consumed any (she had eaten a hazelnut and almond-based cake) and the young man had eaten the same peanut candy the day before, without any reaction (but at the time of the anaphylaxis, he had also eaten a hazelnut cookie). The reactions were very severe with unconsciousness, generalized urticaria, and abdominal pain in both cases, asthma for the 1st patient and a blood pressure drop for the 2nd patient. An emergency team was called for the 1st patient, whereas the 2nd patient was taken to his physician: both were given intramuscular adrenaline. In both cases patients usually tolerated hazelnut, but anaphylaxis occurred in co-factors contexts: uncontrolled asthma and pollen exposure for the 1st patient, intense stress and pollen peak exposure for the 2nd patient. Additional tests allowed to conclude to anaphylaxis to hazelnut involving PR 10 with positive r Bet v1 and r Cor a 1 for our 2 patients. On the occasion of these anaphylaxis, we discuss cases of anaphylaxis involving PR10 reported in the French Allergy Vigilance Network and in the literature, analyzing the importance of co-factors. The severity of these accidents raises the problem of their prevention: should PR 10 hazelnut-sensitized patients be warned of a risk in the presence of co-factors (especially physical effort)? Should our PR 10 allergic patients be systematically equipped with adrenaline auto-injectors? Should we recommend strict avoidance of hazelnuts?" @default.
