- W3091926676 abstract "Background: Minimally invasive liver resection has several patient benefits including fewer major complications and an enhanced recovery after surgery. The surgical robot, equipped with articulating instrumentation and 3-dimensional view, is increasingly employed in hospitals worldwide for minimally invasive liver resection. The robotic platform provides a number of devices that can be used during parenchymal transection of the liver, including ultrasonic, bipolar, and sealing devices alongside robotic clips, hem-o-lok clips , and staplers. Here, we report the outcomes of 60 robotic liver resections during which the articulating Vessel Sealer was used for the main part of the parenchymal transection. Materials and Methods: We included patients who underwent robotic liver resection between August 2014 and September 2018 at our center. Patients were selected for a minimally invasive approach during a multidisciplinary meeting. Data on operative parameters and postoperative outcomes were prospectively collected. Complications were scored during index admission. Primary outcome was major complication (≥ Clavien-Dindo grade III). Results: In total, 60 patients underwent robotic liver resection. We performed 29 minor (≤ 3 segments) resections of the anterolateral segments and 31 technically major resections (resections of the posterosuperior segments or hemihepatectomies). In all procedures, the Vessel Sealer device was used for parenchymal transection. Four procedures were converted to an open laparotomy. Mean operative time was 156±73 min. Median blood loss was 150 mL (IQR 35-300). A major complication occurred in 7 patients. Three patients suffered from bile leakage during their postoperative recovery. Length of hospital stay was 4 (IQR 3-6) days. There was no in-hospital or perioperative mortality. Conclusion: Robotic liver resection is a safe and feasible technique, regardless the segmental location of the lesion. The use of the Vessel Sealer for parenchymal transection is safe and provides adequate postoperative outcomes." @default.
