- W3092016145 abstract "A 69-year-old man with newly diagnosed B-precursor acute lymphoblastic leukemia (B-ALL) had impaired consciousness on day 24 of remission induction therapy, with a Glasgow Coma Scale (GCS) score of E4V4M6. Serum beta-d-glucan (61.0 pg/μl) and Aspergillus galactomannan antigens in the serum and cerebrospinal fluid (CSF) (both ≥5.0) were elevated. Computed tomography showed a nodule in the left lung. Magnetic resonance imaging (MRI) revealed multiple brain lesions, and a brain biopsy confirmed aspergillosis (Figure 1) according to the updated invasive fungal disease definition (Donnelly et al., 2020Donnelly J.P. CSF culture on day 26 was negative. Prophylactic itraconazole was switched to intravenous liposomal amphotericin B (L-AMB, 5 mg/kg/day), but his consciousness deteriorated rapidly on day 45 (GCS E3V3M5). Brain MRI revealed intraventricular rupture of brain abscess (IVRBA, Figure 2). He was deemed unfit for surgical intervention; L-AMB was switched to intravenous voriconazole (12 mg/kg/day). Consequently, his condition and consciousness ameliorated, and brain abscesses were well controlled thereafter. Although he achieved complete remission on day 40, he died of relapsed disease 9 months after the onset of B-ALL.Figure 2Intraventricular rupture of the right temporal lobe lesion.
