- W3092486249 abstract "Background: The peri-implant soft tissue has been proven to be a key success factor in implant treatment in many studies. The gingiva biotype in the lateral mandible is usually thin. We are therefore continuously searching for ways to improve the periimplant soft tissue quality and quantity in this region. Aim/Hypothesis: The aim is to determine if the use of short gingivaformers instead of the conventional healing screw in the 2-staged implant procedure could lead to increased height of peri-implant soft tissue in the lateral mandible. The hypothesis is that a chamber effect could be created. Materials and Methods: 10 patients requiring implants in the lateral region of the mandible were selected.. The height of soft tissue was measured via 2 methods 1. Standardized CBCT 2. Measurement of the full thickness flap with a periodontal probe After measurement of the gingiva height, the implants were intentionally placed 1 mm subcrestally, and presterilized 2 mm gingivaformers were mounted immediately instead of healing screw. During the mounting of the gingivaformers, a squared piece of collagen fleece was fixed. The soft tissue was then sutured tension-free over the gingiva former. A control Panoramic X-ray was made to each patient, to assess the depth of the implant position. Patients were recalled after 3 months and after incision, the existing gingivaformers were replaced with 6 mm ones. After 2 weeks healing time, the exposed part of the gingivaformer was measured with a perio probe. Results: The measured differences were noted and we observed a gain in the soft tissue height of 1-1.5 mm. It is still to be discussed the quality of the soft tissue, which we have obtained in this way. But for that we need to follow-up the cases in the next years. Conclusions and Clinical Implications: This method proved to be a reliable way of increasing the quantity of soft tissue around implants and, given the fact that we need at least 3 mm gingiva height around implants, the authors can foresee that in the future, the whole implantation protocol can be modified and instead of the usual healing screw in the lateral area, we would use special gingiva formers. The incidence of bone resorption in the mandibular lateral area could decrease significantly.
