- W3092505280 abstract "Background: Bile duct injury sustained during laparoscopic cholecystectomy is a serious complication. The repair is a challenge even for surgeons who specialize in hepato-biliary surgery, and the fate of patients and the long-term results are uncertain despite successful reconstruction. Material and Method: There were 744 patients to whom hepatico-jejunal anastomosis has been performed due to injury of the bile duct between 1990-2015. Two groups were defined: 328 (group A) underwent the attempt of repairs in public hospital; 416 (group B) not (gr. B). The aim of study was the evaluation of patients fates after at least three years after reconstruction and disclosure of factors that interfere with the achievement of positive long-term results . Data relating to the effects and the circumstances of repair were adopted as the variables. The factors interfering outcome were established statistically. A P-value < 0.05 was adopted as significant. Results: In 3-year follow-up 119 (36.5%) of patients A, and 246 (59.2%) of patients B were without symptoms. The mean time of asymptomatic after reconstruction in all 744 patients was 5.6 years and 6.1 years, and the mean time of renewing of the biliary anastomosis was 6.8 years (range 3.9 - 14.8) and 7.7 years (range 1.9 - 12.9), in group A and group B, respectively. The analysis showed intra-abdominal infection; failure of local surgeons in reconstructing the bile ducts; cholangitis and female sex were the independent interfering factors of outcome. Conclusion: The study indicates how the level of efficacy of definitive repair is dependent upon the circumstances surrounding the treatment. The benefits of definitive repair in HPB Center were apparent in all aspects of the patient's life; the renewal of bilo-jejunal anastomosis and subsequent surgical or endoscopic interventions were less needed, and asymptomatic periods of time lasted longer (p<0.001)." @default.
