- W3093562648 abstract "Concurrent hyperthermia (HT) has been regularly employed clinically as a radiosensitizer with limited added toxicity. In the setting of particle therapy, superficial and deep thermal therapies have been theorized as excellent adjuncts. However, there is concern for synergistic toxicity, given uncertainty regarding radiobiologic properties of the pairing. With a lack of institutions with capability to deliver both therapies, little clinical data is currently available. We report, herein, the largest institutional experience to date utilizing superficial HT in conjunction with pencil beam scanning proton therapy (PBS-PT). Amidst over 2,000 patients treated with PBS-PT, our institution has completed 50 courses of concurrent PBS-PT and superficial HT in 46 patients. HT was delivered using a superficial microwave-based applicator with a 915 MHz generator and three sizes of wave guide applicators. HT target temperatures were between 40-44ºC. Water bolus temperatures were set between 39-40ºC. HT was delivered twice weekly on non-consecutive days to the target area. Median HT sessions per patient was 10 (range, 4-28), and the median PBS-PT dose was 50.4 Gy (RBE) (range, 30-72 Gy (RBE)). Histologies were heterogeneous, with sarcoma and breast cancers being most common: sarcoma (n = 21), breast cancer (n = 17), squamous cell carcinoma (n = 3), adenocarcinoma (n = 2), esophageal cancer (n = 1), multiple myeloma (n = 1), ovarian (n = 1), vulvar (n = 1), mesothelioma (n = 1), desmoid (n = 1), and ureteral (n = 1). Median follow-up of 8 months (range, 1-31 months) and included 36 patients (72%) with a recurrent malignancy. Grade 3 toxicities were limited to: radiation dermatitis (n = 7), chronic lymphedema (n = 2), and non-healing wounds (n = 2). There were no Grade 4-5 toxicities. At time of last follow-up, 38 patients (83%) remain alive, 39 tumors (78%) were locally controlled, and 28 patients (61%) were free of disease. The combination of superficial HT and PBS-PT was well tolerated and a safe and effective combination in this largest series to date, with acceptable and promising toxicity profiles among a high-risk cohort of patients." @default.
