What’s included? The book begins with a concise but thorough overview of ethics and of the ethical systems that are most useful for decision making, including principlism as well as consequentialist, deontological, and virtue ethics approaches. This section is extraordinarily well written and serves as a basic foundation for readers who may not have formally studied philosophy or ethics, or as a review for those who have. These concepts are then brought in to a deeper consideration of issues that surgeons and other practitioners face every day, such as those related to informed consent, research and innovation, and conflicts of interest. The bulk of the text comprises individual chapters that each address a particular question or problem in pediatric cardiac care, including everything from prenatal diagnosis and abortion to end-of-life care, resource allocation, and transplantation. Each chapter includes a (generally well-referenced) background section on the “state of the science,” recognizing that in order to make a ethically sound judgments we need to begin with the best possible understanding of the clinical options and their likely results. The chapter’s specific dilemma is then subjected to ethical analysis, ending with its author’s recommendations. The chapters are all expertly written, and generally come to conclusions that are well supported by both the scientific data and the ethical analysis. Controversial topics (think: abortion, euthanasia, and care of undocumented patients—but most of the topics are controversial!) are treated sensitively but firmly. There were (only) 2 chapters in which I thought the ethical analysis was not terribly rigorous. In addition, one strayed further into politics than I would have chosen to do, and in another, the author came to a conclusion that I thought presented personal opinion as normative. All the topics will be areas for future discussion and debate, especially as science and culture evolve. Why do I recommend it? Bioethical Controversies in Pediatric Cardiology and Cardiac Surgery can be used as either a text to read from start to finish or as a reference book to which to refer when questions arise (or both). Although its intended audience is one made up of clinicians, it will likely also prove useful for clinical ethics consultants and ethics committee members. Recognizing the fact that in order to make complex ethical decisions, we need to understand both ethics and medical science, the editors have included an ample background of both. This combination is surprisingly uncommon in the bioethical literature, and should be emulated in other specialties.
