However, prior studies have identified significant gaps in radiation oncology residency curricula regarding palliative care. In recent years, theMednet, an online community of physicians whose mission is to “improve knowledge sharing between physicians so that patients get high quality care wherever they are treated,” has become an increasingly utilized resource for radiation oncologists with questions about clinical management. In this study, we sought to identify knowledge gaps in palliative radiation oncology by examining all of the relevant questions posted on theMednet. We searched for all questions posted on theMedNet and tagged with the terms “Radiation Oncology” or “General Radiation Oncology” and “Palliation” on or before January 7, 2020. All identified questions were classified into similar thematic groups. Four measures of social engagement were recorded for each question: the number of times it had been viewed, marked as a “good question,” and answered; and the total number of comments posted in response to any answers. Differences in social engagement between the thematic question groups were assessed using the Kruskal-Wallis and Mann-Whitney U Tests. 161 questions were identified, 147 of which were answered (91%). Seven broad thematic groups were identified: 42 questions (26%) pertained to the decision to treat a patient, 36 (22%) to treatment planning, 26 (16%) to appropriate palliative dose regimens, 19 (12%) to treatment logistics, 10 (6%) to supportive care, 8 (5%) to patient counseling, and 7 (4%) to pre-radiation therapy work-up. The remaining 13 disparate questions (8%) were classified as an eighth thematic group. On average, palliative radiation oncology questions were viewed 497.7 times (range 7 - 4657), marked as a “good question” 2.2 times (range 0 – 29), and answered 1.7 times (range 0 – 10); answers to each question received an average of 0.4 comments (range 0 – 7). Among thematic question groups, questions about dose regimens received the most average views (702.5) and answers (2.1). Questions about treatment logistics were marked as “good question[s]” most frequently (2.9 times on average) and answers to questions about the decision to treat a patient received the most average comments (0.8). On statistical analysis, there were no significant differences in any of the four measures of social engagement among questions in different thematic groups (p > 0.05). We identified several common question themes among palliative radiation oncology questions posted on the Mednet. These results suggest that significant knowledge gaps remain among radiation oncologists in regards to palliative care. Modifications to radiation oncology residency curricula may help reduce clinical uncertainties in the future." @default.
