- W3094604839 abstract "INTRODUCTION: Gastrointestinal (GI) hemangiomas, specifically involving the terminal ileum, are rare. Intussusception infrequently manifests in adults. Thus, ileocolonic intussusception caused by a hemangioma in the adult population is exceedingly rare. CASE DESCRIPTION/METHODS: We report a unique case of a 51-year-old female with a PMH of severe iron deficiency anemia presenting with a three-month history of intermittent crampy abdominal pain without evidence of acute GI bleed or bowel obstruction. Her physical exam was unremarkable and labs were notable for Hgb of 10.4 and severe iron deficiency. She was found to have an ileal mixed capillary and cavernous hemangioma leading to intussusception, requiring laparoscopic resection. A literature review, discussion of this disease, and considerations for management are discussed. DISCUSSION: Though rare, small bowel hemangiomas should be considered in the differential diagnosis of chronic iron deficiency anemia of unknown etiology or acute GI bleeding. If associated with abdominal pain, obstruction or intussusception may be present. X-ray, CT, and MRI can assist in diagnosis. If routine upper endoscopy and colonoscopy are unrevealing, most lesions are visualized with capsule endoscopy and/or double balloon enteroscopy. Small lesions may be treated endoscopically but the majority of hemangiomas require surgical excision to relieve symptoms and prevent further complications.Figure 1.: Endoscopic appearance of the lesion: Ileal hemangioma intussusception extending into the transverse colon.Figure 2.: Histopathological examination of the lesion: Photomicrograph of resected intestine with the pedunculated polyp arising from the submucosa showing benign vascular proliferation throughout the full thickness of the mass.Figure 3.: Radiographic appearance of the lesion (CT A/P): Prominent inflammatory changes in the right lower quadrant with thickening of the distal and terminal ileum with a 1.8 cm fatty lesion that appeared to invaginate into the cecum suspicious for an ileocolic intussusception." @default.
