- W3100273860 abstract "The cardiac extracellular matrix (ECM) provides organisation to the heart, and following myocardial infarction (MI) is altered to compensate for negligible cellular regeneration. A number of ECM biomarkers participate in this process. However, levels of biomarker variation, and their association with infarct size, remains largely unknown. In this pilot study, we examined infarct size in subgroups of patients with consistently elevated or decreased biomarker concentrations across time post-MI. Plasma samples were collected from 23 MI patients on days 1-3 and at six months following MI, and a single sample was collected from 12 healthy volunteers. Using ELISA and multiplexing assays, we measured nine ECM biomarkers (MMP-2, MMP-3, MMP-9, osteopontin, periostin, PINP, TGF-β1, TIMP-1 and TIMP-4). AUC troponin T (hsTnT), a surrogate for infarct size, was calculated by combining daily measurements collected during hospitalisation. When compared to the interquartile range of healthy volunteers, four biomarkers had a subgroup of patients with consistently elevated levels across time (MMP-2, MMP-9, PINP & TIMP-1) and two had a subgroup with consistently decreased levels (MMP-3 & periostin). Infarct size was significantly smaller in patients with decreased levels of either MMP-3 (median, 909.5 [IQR, 718.5-2,476]ng/L versus 5,709 [1,670-10,662]ng/L, n=6) or periostin (1,003 [628.5-2,589]ng/L versus 5,709 [1,949-9,291]ng/L, n=8) when compared to patients with typical levels of these biomarkers (all, p<0.05). Patients with consistently decreased levels of MMP-3 or periostin had lower levels of AUC hsTnT, suggesting a link between these biomarkers and reduced residual damage post-MI.
