- W3100655209 abstract "To the Editor: Our discussions with colleagues in the medical education community have revealed that some medical schools are administering multiple-choice summative assessments of competence online and without proctoring—or with web-based remote proctoring that does not involve high-security proctoring protocols—during the COVID-19 pandemic because of resource limitations, cost-effectiveness, and other reasons. This practice implies that inferences regarding the validity of test scores of individual examinees rest in part upon the examinee adhering to the institution’s honor code policies. We concur with Nasca and Heard that only through “transparency and accountability can we gain, regain, and reaffirm the trusting relationship with the public.”1 Thus, in the interest of transparency for the end users of test results (such as residency program directors) and ultimately the public, and in fairness to all examinees, we believe medical schools should annotate results of summative assessments that deviated from the originally stipulated conditions of in-person proctoring when they report these results in the Medical School Information section of all Medical Student Performance Evaluations (MSPEs) issued for students in affected medical school classes. These annotations should indicate that the decision to administer summative assessments unproctored or with web-based proctoring was a measure the school decided was appropriate under these unprecedented circumstances, and that individual examinees did not have a role in determining how these tests would be administered. This would make it clear that the annotation is intended to demonstrate transparency on the part of the institution making the disclosure rather than a reflection on an individual examinee’s professional conduct. (This annotation would not apply to any summative assessments deliberately designed as “open book” exams.) We believe including this annotation is the responsibility of individual schools. It respects the value of transparency and equitable treatment of all in the testing process, and it is consistent with both Standard 6.3 in Standards for Educational and Psychological Testing2 and the recommendation for MSPEs to have “a level of standardization and transparency that facilitates the residency selection process.”3 Transparency in this regard will ultimately serve to protect graduates’ collective interests and enhance public trust in institutions. In contrast, a lack of transparency from individual medical schools may raise ethical concerns, and we might anticipate future legal challenges following the residency selection process, alleging unfair advantage or disadvantage to individual students/graduates who took such exams. In this regard, specific clarifying guidelines from organizations such as the Association of American Medical Colleges would be useful to all medical schools." @default.
