- W3102001559 abstract "近年，“Powered exoskeleton”と呼ばれる動力付外骨格型機器（以下，外骨格）が実用化され，脊髄損傷者の歩行再建に有用となる可能性が示されている.しかしながら，安全性の観点から外骨格について調べた研究報告は数少ない．我々は安全な外骨格の設計と運用に資するための基礎的知見を得ることを目的に，外骨格を用いたリハビリテーションの現状を調べ，そのリスクを分析した．まず文献調査の結果によれば，外骨格の有害事象報告には皮膚トラブルや機器の不具合の件数が多かった．転倒の件数は多くはなかったが，これは介助者の支援行動の有用性を示唆するものであった．次にリハビリテーション施設における2種類の外骨格を用いた歩行訓練を観察した結果，理学療法士2名の接触介助と近接監視が外骨格を装着した対麻痺患者の転倒を防止し，安全性の確保は介助者の支援行動に依存していることを裏付けた．一方，介助を担う理学療法士の不安定な歩行や心理的・身体的負担を示唆する行動特性が見られ，機器の適合性・使用性等の問題もあった．最後にリスクアセスメントの結果からは，複数の重要危険源に対する工学的保護方策の適用はリスク低減に寄与するが，転倒等の危険源に関しては使用者に依存する残留リスク方策と組み合わせなければ，リスク回避が困難であることが示された．外骨格の設計と運用にあたっては，装着者・介助者の使用を前提に，工学的手段と人的支援との包括的なリスク低減方策が必要と考えられた．" @default.
