- W3105339984 abstract "Platypnea-orthodeoxia is a rare syndrome defined by dyspnoea and hypoxemia exacerbated by an upright posture. The syndrome is reported to be underdiagnosed, yet diagnosis is critical as the underlying cause is often treatable, leading to profound improvements in the patient’s quality of life. Our first case is a 56 year old lady with a two month history of progressive dyspnea and fatigue (ECOG 1-2), with platypnea and orthodeoxia saturating at 97% SpO2 on room air supine and 84% on standing. Although a transthoracic echocardiogram (TTE) with bubble study was unremarkable, a transoeophageal echocardiogram (TOE) revealed an atrial septal aneurysm and a large patent foramen ovale (PFO) with right to left shunt enhanced in the sitting position. The PFO was closed percutaneously. On 1 month and 3 month follow-up the platypnea-orthodeoxia was noted to be resolved and the patient was feeling entirely asymptomatic (ECOG 0). Our second case is a 26 year old otherwise healthy lady who presented with progressive dyspnea leading to limitation of daily activities, recurrent postural syncope, platypnea, orthodeoxia and postural cyanosis. A TTE demonstrated a large 15mm secundum atrial septal defect. After percutaneous closure, the dyspnea, syncope and postural cyanosis completely resolved. Platypnea-orthodeoxia syndrome is a debilitating yet underdiagnosed condition. As cardiac platypnea-orthodeoxia syndrome can be successfully treated with percutaneous closure resulting in symptom relief, the diagnosis requires active consideration in patients suffering from dyspnea." @default.
