- W3105352105 abstract "The extent to which sinus node (SN) remodelling in the right atrium (RA) due to atrial fibrillation (AF) remains incompletely defined. 25 consecutive patients with AF (11 paroxysmal AF) and 25 age-matched control patients with supraventricular tachycardia (SVT) underwent high-density electroanatomic multipolar mapping prior to ablation during sinus rhythm (SR). SN location, early activation sites (EAS) and sinus break-out (SBO) were measured. Global and regional crista terminalis voltage, conduction velocity (CV) and complex signals were determined in SR and proximal CS pacing at 600ms and 300ms. Groups were age-matched with mean age of 59±9 years and 24 (48%) were male. There was progressive SN remodelling from the control to paroxysmal to persistent AF characterised by lower SN voltage ((2.19±0.51 vs 1.75±0.34 vs 1.31±0.56mV, p<0.001), caudal shifts (5.9±7.2 vs 9.9±2.6 vs 15.1±5.5mm, p<0.001), reduced number of EAS (4.1±2.2 vs 3.3±1.8 vs 1.6±1.5, p=0.002) and longer EAS-SBO conduction times (9.6±3.6 vs 13.8±5.3 vs 18.9±5.9 ms, p<0.001). AF patients had significantly lower global voltage, CV and increased complex signals and rate-dependent change at the crista terminalis. This study demonstrates that patients with AF have greater SN remodelling characterised by caudal shift in the pacemaker complex, smaller SN activation area, lower SN voltage, conduction slowing and anatomically-determined latent substrate. These findings highlight progressive remodelling in AF with implications for early treatment in AF." @default.
