- W3113806952 abstract "Sir: We appreciate the authors’ comment on our clinical trial.1 The authors note that it could have been suitable to have the study focused on grade II and above fat necrosis according to the classification reported by Lie et al.2 We observed in our clinical practice that approximately half of our microsurgical breast reconstruction patients presented small nodules in the periphery of the flaps attributed to fat necrosis. Thus, we aimed to evaluate whether fluorescent angiography with indocyanine green could even reduce the smallest nodules of fat necrosis.1 We recorded 18 patients with fat necrosis in the overall sample. Fourteen cases corresponded to grade I nodules, and only four cases corresponded to a higher degree, which belonged to the clinical evaluation group. No grade II or III fat necrosis cases were recorded in the fluorescent angiography group. We agree with the authors that the larger the necrosis, the more it will adversely affect the result of the breast reconstruction. However, our experience was that fat necrosis nodules smaller than 2 cm could also affect the well-being of the patient. Anyway, given our results, it is likely that fluorescent angiography would also have resulted in a significant decrease in the incidence of grade II (and above) fat necrosis, but a much larger sample size would have been necessary to achieve a statistical difference. We did not exclude patients because of the timing of the reconstruction. Thus, the patients were recruited as they presented to our clinics; thus, they may represent a random sample of a population subject to actual breast reconstruction. The timing of reconstruction may influence the satisfaction scores, and we analyzed this variable using a Fisher’s exact test 2 × 2 contingency table. As shown in Table 1, the value of p was 0.136. Therefore, there were no significant difference either for immediate or delayed reconstruction.
