If everyone could be relied upon to work to the same high standards as the Newcastle group I might support their position, but the fundamental problem, as we have learned from bitter experience, is that not everyone does practise to such standards. I am pleased to see Lahoud supporting my position [4]. I am sorry that the Glasgow group have used my letter as an opportunity to be so critical about intravenous midazolam for conscious sedation in dentistry. No drug is perfect, but it is a simple, straightforward, economical technique which can be used by the single-handed practitioner in dental practice after appropriate training and working to sensible standards. In criticising the technique Leitch et al. [3] refer to the extensive literature on the complications of sedation for endoscopy, but that is hardly relevant. Most of the problems in endoscopy relate to polypharmacy, or failure to recognise the consequence of administering sedative drugs to (often elderly) patients with significant intercurrent disease or physiological disturbance, or both. The young, healthy, pathologically anxious dentophobe is an entirely different clinical challenge and intravenous midazolam is very useful. Patient maintained sedation may well be a long-term solution, but it is clearly far from ready for use in the primary care setting where the majority of such patients will always have to be managed." @default.
