- W3123924652 abstract "Outcome research has demonstrated statistically significant improvements in both pain and function following anterior cruciate ligament reconstruction (ACLR), but statistical significance doesn't always correspond to clinical outcomes. Accordingly, the purpose of this study was to define the time-dependent achievement of the minimally clinically important difference (MCID) and substantial clinical benefit (SCB) after ACLR, as well as factors predictive of achievement. A prospective institutional registry was queried for patients receiving primary ACLR between January 2014 and April 2017. Exclusion criteria were revision procedures, concomitant procedures, or lack of consecutive PROM completion at 6-, 12- and 24-months. PROMs of interest included the IKDC and all KOOS subscales. Cumulative probabilities and odds of achieving MCID and SCB were calculated using Kaplan-Meier survival curve and Weibull parametric analyses, respectively. A total of 144 patients met inclusion criteria, with average age 30.9±12.8 years and average BMI 25.5±4.6 kg/m2. MCID achievement ranged from 52.0% to 73.5% at 6-months and 82.1% to 97.3% at 12-24 months. SCB rates ranged from 40.0% to 65.5% at 6-months and 74.9% to 96.7% at 12-24 months. Participation in preoperative exercise routines was predictive of early achievement of MCID on the IKDC (OR: 0.63, p=0.01), whereas longer symptom duration was associated with delayed achievement of MCID (OR: 1.32, p=0.02) and SCB (OR: 4.14, p=0.15) on IKDC. Older age was associated with delayed achievement of SCB on KOOS-Sport (OR: 1.14, p=0.007) and KOOS-Symptoms (OR: 1.05, p=0.02), as well as patients receiving bone-tendon-bone (BTB) allografts on KOOS-ADL (OR: 1.72, p=0.003) and KOOS Sport (OR: 1.42, p=0.001). The majority of patients achieved MCID and SCB on IKDC and KOOS measures within 6-months. Older patients, greater preoperative symptom duration, and patients receiving BTB allografts experienced delayed MCID and SCB achievement, while those involved in preoperative exercise routines exhibited earlier achievement." @default.
