- W3126057050 abstract "Background: Cardiac rehabilitation (CR) programs are associated with decreases in total mortality;cardiac related mortality and morbidity;and one-year re-admission in individuals diagnosed with heart disease (HD) Despite the known health benefits of and recommendations for CR, CR programs are highly underutilized with 70 to 80 percent of all patients who require CR not utilizing the program This study sought to explore how the intersections of: (1) social and economic contexts of the health care delivery;(2) social and personal contexts of patients' individual lives;and (3) patients' navigation through their chronic illness journey, shape the experiences of patients with heart disease with respect to utilizing CR programs Methods: This qualitative study conducted semi-structured face-to-face interviews with adult patients diagnosed with a form of HD (n = 19) Interviews lasted 60 to 90 minutes and explored the experiences of patients with using a center based CR program from the time of obtaining a referral, through attending weekly in-person exercise sessions, to completing the prescribed course Data analysis involved qualitative thematic analysis and constant comparative methods Results: Eight males and eleven females were included in the analysis Two dominant themes were generated related to barriers/facilitators to program utilization;familial responsibilities and fear of exercising after a cardiac event requiring hospitalization Familial responsibilities included having to work to meet the financial needs of the family and/or caregiving responsibilities including cooking, cleaning, and childcare For some participants (n = 7) these responsibilities posed challenges for being able to access a center based CR program While for others (n = 5), being able to use a center based CR program provided them with the opportunity to 'escape' their familial responsibilities Participants (n = 4) also discussed fears of exercising after a cardiac event that required for them to be hospitalized Being able to exercise in a center based CR program with specialized nurses, cardiologists, and advanced monitoring equipment provided participants with the 'courage' and 'confidence' to exercise Conclusion: The provision of a center based CR program provided both opportunity and challenge for patients being able to obtain CR care As we move forward in redesigning CR programs to fit the new normal of care in the context of COVID pandemic, we ought to also be cognizant of the individual needs of patients, those who may or may not benefit from the current dominant center based approach to CR" @default.
