- W3126644107 abstract "The history of cancer therapy is replete with examples of new treatment modalities that were adopted with great enthusiasm, became the standard of care, but were ultimately discarded. The surgical treatment of malignancies that involve the mandible or the cervical lymphatics is a case in point. Treatment was rather stereotyped and rigidly applied for much of this century. This rigid philosophy of treatment is no longer the standard. As we have refocused our interest on these cancers in recent years, we have reached a better understanding of the rather predictable patterns of tumor growth and metastasis. Modern imaging technology has enabled us to improve the accuracy of preoperative staging. As a result, surgical treatment has become far more individualized, and it may include either less-than-radical surgery or immediate reconstruction in selected cases. Many patients can be offered an oncologically sound surgical resection with minimal anatomic, physiologic, and cosmetic consequences. Nevertheless, in these days of rapidly escalating health care costs, we can no longer accept at face value tests and treatments that translate into millions of dollars for our patients. Although they all show a great deal of promise, some of the diagnostic imaging and conservative surgical techniques discussed in this article have yet to be subjected to properly controlled, prospective clinical trials to assess survival, recurrence, morbidity, and quality of life after treatment.
