- W3127872782 abstract "Omphalocele ratios have been shown to predict postnatal outcomes such as ability to achieve primary closure in fetuses with omphalocele. The objective of this study was to determine if omphalocele ratios or other biometric characteristics can function as predictors of genetic abnormalities. This is a retrospective cohort study of patients presenting to a single tertiary referral center from 2004-2019 with fetal omphalocele diagnosed on prenatal ultrasound. For each subject, images were reviewed and omphalocele contents, maximal omphalocele diameter, and ratios calculated for all initial studies with archived measurements. The primary outcome was a composite of abnormal genetic screening/testing results in the pre- or postnatal period. Omphalocele ratios, size, and presence of extracorporeal liver were evaluated as predictors of the primary outcome. A logistic regression model was performed to determine the presence of a statistically significant association (p < 0.05) with the predictor and outcome. Where an association was demonstrated, receiver operating curve analyses were conducted to determine how well test cut-offs predicted the primary outcome. Eighty-seven confirmed diagnoses of fetal omphalocele with images and available genetic outcomes were included in this analysis. Mean maternal age was 31.8 years (± 6.4) and median gestational age was 21 weeks 6 days (IQR 7.5) at initial sonogram. Abnormal genetic results were detected in 18 cases (20.7%). Omphalocele size cut-offs and ratios were poor predictors of genetic abnormality, with omphalocele to abdominal circumference ratio performing best among ratios. The absence of extracorporeal liver performed slightly better in predicting genetic outcomes when compared with size or ratios. Prenatal sonographic omphalocele characteristics are poor predictors of associated genetic abnormality. Due to the high risk of underlying genetic anomalies in cases of fetal omphalocele, genetic testing is strongly recommended for all patients regardless of omphalocele size, ratio, or the presence of extracorporeal liver." @default.
