Second, we need to make immunosuppression-free transplantation possible. Many strategies have been implemented to increase the donor pool, yet the gap between the demand and the offer remains too wide and, as a consequence, the number of patients dying on the waiting list keeps rising. Tolerance has been pursued for decades, yet it remains an elusive goal outside liver transplantation, where the weaning off immunosuppression appears a safe option in selected cases. As the field of regenerative medicine has shown immense potential to address these challenges, transplant medicine should embrace the opportunity to join forces and partner with it. Regenerative medicine will enable organ-on-demand whereby patients will receive organs that will not be rejected and in a timely fashion. This will make registration in the waiting list and antirejection medications unnecessary and, as the new organs will be implanted immediately after fabrication, ischemia-reperfusion injury secondary to organ preservation will not be a problem anymore. The journey has just begun. As organ transplantation has become the standard of care for many end-stage diseases, its success has become its pitfall. The demand for organs has been steadily increasing in the last years and will continue to do so as chronic noncommunicable diseases such as hypertension, diabetes, and obesity progress to a pandemic level (Perico and Remuzzi, 2012). The average waiting time for patients to receive an organ has increased, with an overwhelming majority waiting 5 years or more. Correspondingly, the mortality rate of those on the lengthening waiting list is rising while the overall probability of receiving an organ in the appropriate time is dropping (Delmonico and McBride, 2008). As a consequence of the steep demand and plummeting supply of organs, initiatives to increase the donor pool have been implemented but have failed to match the demand. Such initiatives include transplanting organs across immunological barriers, paired donations, and expanding the criteria of transplantable organs from deceased and living donors (Vincenti et al., 2016; Massie et al., 2014; Bozek et al., 2018; Orandi et al., 2019).
