- W3129059626 abstract "This study aimed to assess factors that influence patients’ decisions in accepting prenatal invasive testing following genetic counseling for positive fetal aneuploidy screening. A retrospective cohort study of women with high risk for fetal aneuploidy with genetic counseling from Jan. 2012 to Dec. 2016. Demography, indications for genetic counseling and rates of invasive testing were compared between those who accepted prenatal invasive testing (PIT) and those with non-invasive prenatal screening (NIPS). The variables were analyzed using chi-square, Fisher exact test and multiple logistic regression analysis. 2373 pregnant women received genetic counseling for increased risk of fetal aneuploidy during the study period, 321 women had PIT (13.5%). Amongst these, women > 35 years old were more likely to accept PIT than < 35 years old (20.7% versus 11.5%, p < 0.001). Asian women were most likely to accept PIT, 27.7% followed by white, non-Hispanic Black and Hispanic women at 18.0%, 12.1% and 11.7% respectively, p = 0.002. There was a direct relationship between the number of indications for genetic counseling and likelihood of acceptance of PIT. Women with one indication, 11.5% accepted PIT; with two indications, 17.0%, and three or more indications, 29.2%. The most common indication for PIT was cystic hygroma (RR 7.5, 95% CI 3.12-8.76 p < 0.001). Other indications such as shortened fetal long bones, thickened nuchal fold, echogenic bowel, two-vessel umbilical cord, increased nuchal translucency were statistically significant, RR of 4.0, 3.3, 3.1, 2.7, and 2.7 respectively. However, women with abnormal maternal serum analytes were less likely to accept PIT (RR 0.8, 95% CI 0.7 – 0.96, p = 0.017) compared with other indications. This study showed that several factors such as age, race and ethnicity, and cumulative number of indications for genetic counseling influenced patients’ decision to undergo prenatal invasive testing among women with increased risk for fetal aneuploidy.View Large Image Figure ViewerDownload Hi-res image Download (PPT)" @default.
