- W3133431150 abstract "BACKGROUND:Fibroid uterus or leiomyoma is one of the most common benign uterine conditions. It can present with a variety of symptoms ranging from asymptomatic fibroid to abnormal uterine bleeding. Fibroid growth depends on hormones mainly oestrogen and progesterone which also causes endometrial thickening. There is a 0.4% chance of incidental finding of malignancy in women with fibroid with or without endometrial thickening. AIMS OF THE STUDY:Our study aims to correlate between endometrial thickness and abnormal histopathology in women with fibroid.METHODS:394 women in the reproductive and premenopausal age group who were diagnosed with fibroid uterus were recruited for the study. The endometrial thickness was measured by transvaginal scan and the endometrial biopsy by Pipelle biopsy or surgical biopsy was followed up.RESULTS:4 out of 394 women had abnormal endometrium (1.01%). The mean endometrial thickness in women with normal endometrium was 9.9mm, and the mean endometrial thickness in women with abnormal endometrium was 14mm which was not statistically significant (p value 0.12).CONCLUSION:1. The mean endometrial thickness in premenopausal women with a normal histopathology (proliferative, secretary, hormone effect, decidualization) was 9.84±5.32 mm. While women who had an abnormal histopathology (hyperplasia, adenocarcinoma and endometrial stromal carcinoma) was 14.±5.22mm. This difference was not statistically significant.2. There is no correlation between endometrial thickness and BMI, parity, number of fibroid or uterine size in premenopausal women with fibroid.3. There was no correlation between the endometrial thickness and bleeding pattern.(Endometrial thickness of normal bleeding was10.71±6.15mm and endometrial thickness of heavy menstrual bleeding was 9.68±5.09mm , p value 0.123).4. There is statistically significant correlation between endometrial thickness and associated ovarian cyst (Endometrial thickness 11.15± 5.13 mm, p value 0.034) or a previous history of myomectomy (Endometrial thickness 14.8±9.31mm, p vale 0.023).5. There is no correlation between endometrial thickness and comorbidities like diabetes or hypertension. (p value 0.923).6. The most common endometrial pathology in women with fibroid is secretory endometrium.(38.5%).7. The incidence of endometrial hyperplasia in premenopausal women with fibroid is 0.253% (mean E.T 11.67±2.88).8. The incidence of endometrial adenocarcinoma in premenopausal women with fibroid is 0.507%. (mean E.T 18±4.24)." @default.
