- W3133846119 abstract "Aims & Objectives: To evaluate the use of hemostatic blood products (platelets, plasma and cryoprecipitate) and medications (aminocaproic acid, tranexamic acid (TXA), and Factor VIIa) in children supported by extracorporeal membrane oxygenation (ECMO). Methods: We conducted a retrospective observational study using the Pediatric Health Information System (PHIS) database. We included all children ages 0-18 years supported by ECMO from 2012-2017. Data was only included for a patient’s first ECMO course. Results: ECMO was employed in the care of 9193 children at 51 PICUs in the United States. 55% were male and the median (IQR) age was 0 (0-2) years. Platelets were transfused on 49% of the ECMO days, plasma on 33% and cryoprecipitate on 17%. The proportion of patients receiving platelets during ECMO support was trending down (from 57% on ECMO day 1 to 39% on ECMO day 28) whereas the use of plasma and cryoprecipitate precipitously decreased after first few days (from 75% to 23% and from 43% to 8% on ECMO day 1 to ECMO day 4, respectively) and remained stable thereafter. 7% of children received aminocaproic acid, 22% received TXA and 3% received Factor VIIa. Notably, the use of TXA increased from 2012 to 2017 whereas the transfusion of platelets downtrended. Conclusions: Children supported by ECMO are exposed to a significant number of hemostatic blood products. Hemostatic medications, such as TXA, are being used more frequently. Given the known morbidity and mortality associated with hemostatic blood products, studies are warranted to evaluate the appropriateness of transfusion strategies." @default.
