- W3135658520 abstract "Aims & Objectives: Nephrotoxic medications are responsible for a large proportion of pediatric AKI. This study aims to estimate the incidence of AKI after exposure of nephrotoxic drugs (END) in children admitted in a PICU of a resource limited country. Methods: This was a retrospective file review. Patients were grouped according to the presence or absence of AKI, and those with pediatric Risk Injury Failure Loss End stage renal disease (pRIFLE) criteria and severity of “risk” or higher (a 1.5 increase in serum creatinine from baseline or a urine output of < 0.5mL/kg/hr for 6hr) were considered to have AKI. The baseline measure of renal function was defined as the first serum creatinine measurement within 24 hours of ICU admission. The primary outcome was the development of AKI with the degree of nephrotoxic exposure. Clinical outcomes, such as PICU length of stay, duration of mechanical ventilation, need for renal replacement therapy were studied. Results: Nephrotoxin usage was common in our study population. The most commonly administered nephrotoxins were Vancomycin (n = 619, 45.2%), Colistin (n=554, 40.3%) and Ketorolac (n=440, 32.2). Acute kidney injury (AKI) occurred in 36.2% of children who received ≥ 3 nephrotoxins and 29.3 % of those who received less than three medications. Severe AKI rates were similar between those who were and were not exposed to a high nephrotoxin burden. AKI was associated with prolonged length of PICU stay. Conclusions: Children in the PICU are frequently exposed to a large nephrotoxin burden and this is associated with higher morbidity." @default.
