- W313630688 abstract "単クローン性免疫グロブリン関連腎症は，単クローン性の免疫グロブリン分子やパラプロテインが糸球体に沈着する疾患群で，電顕像で細線維ないし微小管構造の沈着物を呈する群と細顆粒状の沈着物を呈する群に大別される。前者の代表的疾患がアミロイドーシスで，後者の疾患群にRandall 型単クローン性免疫グロブリン沈着症（monoclonal immunoglobulin deposition disease; MIDD）と2004 年にNasr らの報告したproliferative glomerulonephritis with monoclonal IgG deposits（PGNMID）がある。PGNMIDの沈着物はMIDD のように電顕像で細顆粒状であるが，免疫複合体型糸球体腎炎類似の沈着分布様式を呈する。Nasr らや当科での検討から，腎生検での頻度は約0.2％程度で，MIDD よりも稀な疾患群であると推定される。発症年齢も20～81 歳と成人例が多かったが，小児例も2013年の腎臓学会東部会や2014 年日本小児腎臓病学会で報告された。また最近，単クローン性IgA 鎖やIgM 鎖，軽鎖のみの沈着症も報告され，疾患概念が拡大してきている。PGNMID の確定診断には，光学顕微鏡，軽鎖を含む免疫染色，電子顕微鏡を駆使した検討が重要である。" @default.
