• W3137543417 abstract "Agomelatine exhibits a pharmacological profile as a melatonin agonist, 5HT2C and 5HT2B antagonist with little or no effect on most other receptor types. Preclinical studies have suggested that the melatonin agonist and 5HT2C antagonist properties are essential for its “antidepressant-like” effects. It is, therefore, the first exemplar of a potential new class of antidepressant agents. In an extensive suite of clinical trials, it has demonstrable efficacy in the short-term management of major depressive episodes arising in the context of unipolar depressive disorder. Efficacy for the treatment of depressive episodes in bipolar disorder is not as well established, although switching to hypomanic/manic episodes appears to be low compared to other classes of antidepressants. In the elderly and for use in the prevention of relapse in the long-term, evidence for the efficacy of agomelatine remains to be firmly established in appropriately designed clinical trials. The medication does not appear to be associated with a withdrawal syndrome on abrupt discontinuation and it exhibits a generally good tolerability profile during use, including a low propensity to cause sexual dysfunction. It can be associated with an elevation of liver function tests during the first few weeks of treatment which may necessitate cessation of treatment. The drug has not been accompanied by serious effects on overdose, although the experience is limited.
