- W3138378462 abstract "Anaesthesia for paediatric ENT covers a wide range of procedures from the simple minor ear operation to the very demanding anaesthesia for endoscopy on a small baby. Adenotonsillectomy, one of the commonest ENT operations, still has significant morbidity and mortality, usually due to postoperative haemorrhage, which should always be taken seriously. Equally important is the risk of sedating a child, restless due to hypoxia, in the postoperative period. Many ENT patients will have upper airways obstruction so they should be carefully assessed preoperatively and the dangers of general anaesthesia and muscle relaxants realized. Endoscopies in babies and young children should, where possible, be undertaken by paediatric anaesthetists as they can be the most difficult of all anaesthetics, particularly in the presence of obstructive laryngeal pathology. Being suitably prepared with a wide range of tubes, and being thoroughly familiar with endoscopes likely to be used, is essential for safe anaesthesia. Microlaryngeal surgery using the carbon dioxide laser is now being undertaken in an increasing number of centres. It has particular advantages in the small paediatric airway; however, because of the special risks and precautions necessary, as with other endoscopic procedures it should be performed by appropriately trained personnel. Finally, it should always be remembered that nowhere in anaesthetic practice is the risk of hypoxia and its serious consequences more real than in paediatric ENT." @default.
