- W3141286989 abstract "Accurate estimations of fetal weight (EFW) are essential to guide antenatal management and to determine the optimal timing and mode of delivery. This study seeks to facilitate objective assessment of current practice in the routine setting. Standards were selected to either match or exceed those noted by Francis, Tonks and Gardosi (2011) in their review of EFW accuracy: ≥ 96% EFW within 20% of birthweight (BW); ≥ 90% EFW within 15% of BW; ≥ 73% EFW within 10% of BW. All obstetric growth scans performed from 24+0 weeks gestation reported at Hull University Teaching Hospitals between 1 June 2019 and 30 June 2019 were reviewed. Where patients had further growth scans at a later date within the same pregnancy, these were also reviewed. All cases where delivery occurred within 14 days of the ultrasound examination were included. BW of each infant was identified through the Lorenzo (computerised record) system. The margin of error was calculated using the EFW Audit Tool provided by the Perinatal Institute which adjusts for the time interval between ultrasound examination and date of delivery (adjusted EFW or EFWa). In addition to an overall analysis, sub-analyses were performed across the following groups: 1. Ultrasound within three days of delivery; 2. Ultrasound beyond 37+0 and 40+0 weeks; 3. Class II+ obesity; 4. EFW ≥ 4kg. A total of 1,054 growth scans were reviewed comprising 811 individual patients of which 497 had an examination within two weeks of delivery. Overall: 1. 98.6% EFWa within 20% of BW; 2. 95.0% EFWa within 15% of BW; 3. 78.5% EFWa within 10% of BW. All standards met with mean error of 6.4% (standard deviation 4.8%). Within the subgroup analyses, all standards were met with the sole exception of EFW ≥ 4kg within the narrowest error margin of error (+/- 10%). A re-audit is currently underway. Within the local department, ultrasound calculates EFW within an acceptable degree of accuracy and in line with or exceeding the results of other reviews. As such, it is reasonable to rely upon these measurements as part of a wider obstetric assessment in order to guide antenatal management." @default.
