Search for more papers by this author Originally published16 Jan 2018https://doi.org/10.1161/CIRCULATIONAHA.117.030854Circulation. 2018;137:314To the Editor:We read with great interest the article by Flyer and colleagues1 about the study of adenosine on atrioventricular nodal conduction in pediatric and young adult patients after heart transplantation, which establishes both safe and effective starting doses (25 μg/kg; 1.5 mg for patients ≥60 kg), in addition to a stepwise therapy escalation plan that avoids prolonged bradycardia and has a great effect on the use of adenosine in patients after heart transplantation with supraventricular tachycardia. We have a suggestion about this study. After heart transplantation, atrioventricular node is denervated; however, during the supraventricular tachycardia episode, activation of sympathetic activity through the hormonal system still occurs. This action may change the electrophysiological properties of the atrioventricular node. The Flyer et al study1 is conducted in normal sinus rhythm instead of supraventricular tachycardia. The results cannot represent safe and effective doses for terminating supraventricular tachycardia completely.
