Or inbreeding. OK, no surprise there. But nor will you see anything about getting your money’s worth. And if healthcare reform done right is supposed to be about getting the biggest bang for all the bucks we spend on the quest for health and longevity, then maybe some sort of money-back guarantee should be in there.It’s an argument payers have long put up when it comes to drug therapies: Show us that it works, and we’ll gladly come back for more. Proof that “it works” can mean anything, from showing that people who stay on a medication regimen have better health outcomes because of it demonstrating fewer long-term health issues in a population as a result of an investment made today in a therapeutic regimen. Some outcomes are easier to quantify than others, but risk sharing — the notion that drug developers will have to give back something if a therapy does not deliver the expected result — is an idea we’ll soon hear a lot more about.For years, risk-sharing contracts between manufacturers and payers have enjoyed popularity in Europe, where large government purchasers have the critical mass to demand it. Now, as Senior Contributing Editor Bob Carlson reports in our cover story, the practice is gaining attention in the United States. As high-cost biologics enter the market and payers ask their makers to demonstrate the value of their products, a handful of manufacturers are pointing to unmet medical needs as an indicator of their worth. In other words, “If a therapy succeeds where others did not, you’ll reap a return on the investment; if not, we’ll make you whole.”As a sales technique, that will take time to gain traction among manufacturers. But for payers that have long complained about the introduction of pharmaceutical products that are only marginally better than what was already out there, it has to be worth a long look.
