- W3146059621 abstract "Ectopic pregnancy (EP) is a significant cause of maternal morbidity and mortality. Majority of the EPs occur within the fallopian tubes, with a small percentage occurring at other non-tubal sites. A tubal stump pregnancy is an ectopic pregnancy that occurs within the remnant of the fallopian tube following previous salpingectomy. Such spontaneous pregnancy remains a rare event, with an estimated incidence of 1.16% of all EPs, with only a handful of cases previously described in literature. We present two cases of spontaneous tubal stump EP diagnosed on early pregnancy ultrasound following ipsilateral salpingectomy for tubal ectopic pregnancy 6 months prior. The two cases had a serum hCG of 4,049mIU/ml and 7,122mIU/ml as well as a mean sac diameter of 8.1mm and 8.6mm respectively, with evidence of hemoperitoneum in the former case and a fetal heart present in the latter case. Both cases underwent primary surgical management with laparoscopic techniques, with one of the cases having significant hemorrhagic complications requiring blood transfusion intraoperatively. Their postoperative recovery was unremarkable, with serial hCG returning to normal levels. Notably, one patient eventually had a successful spontaneous pregnancy outcome. In conclusion, recurrent ipsilateral EP in the tubal stump following salpingectomy remains a rare but possible event. It is important not to allowed the previous history of an absent tube to cloud clinical judgment. A high index of suspicion is required for an early and accurate diagnosis to be made, thus avoiding significant maternal complications secondary to rupture and hemorrhage.
