- W3146688483 abstract "Shenoy et al have eloquently highlighted the significant clinical challenge facing today's urologist in performing cavitating surgery on anticoagulated patients. Leaving aside the potential cardiovascular and cerebrovascular risks associated with ceasing anticoagulants, the urologist also must treat troublesome postoperative bleeding when anticoagulants are begun again. Re: Raj et al.: Stopping Anticoagulation Before TURP Does Not Appear to Increase Perioperative Cardiovascular Complications (Urology 2011;78:1380-1384)UrologyVol. 80Issue 1PreviewThe contemporary urologist is often pressurized by the cardiology fraternity to perform transurethral resection of the prostate (TURP) without discontinuing anticoagulant drugs. Anesthesiologists who were until recently averse to administering regional anesthesia in anticoagulated patients now consider aspirin safe compared with drugs such as clopidogrel or warfarin. The potential for hemorrhage from the raw TURP bed in a setting in which effective reversal of the anticoagulation effect using drugs is not possible (unlike with heparin) or where platelet concentrate is a scarce commodity, has led to several less than ideal solutions to the problem of bladder outlet obstruction such as continuing medical therapy for benign prostatic hyperplasia despite a lack of an adequate response or retaining an indwelling catheter for several months after an acute cardiac event leading to discomfort and related complications.
