While, the diagnostic criteria ofprediabetes are not uniform across various internationalprofessional organizations, it remains a state of high riskfor developing diabetes with yearly conversion rate of5%-10%. Observational evidence suggests as associationbetween prediabetes and complications of diabetessuch early nephropathy, small fiber neuropathy, earlyretinopathy and risk of macrovascular disease. Severalstudies have shown efficacy of lifestyle interventionswith regards to diabetes prevention with a relative riskreduction of 40%-70% in adults with prediabetes. Whilethere is increasing evidence to prove the efficacy ofpharmacotherapy in prevention of diabetes in adults withprediabetes, pharmaceutical treatment options otherthan metformin are associated with adverse effects thatlimit their use for prediabetes. There are no reports ofsystematic evaluation of health outcomes related toprediabetes in children. The effects of pharmacotherapyof prediabetes on growth and pubertal developmentin children remains unknown. Secondary interventionwith pharmacotherapy with metformin is advocated forhigh-risk individuals but criteria for such considerationbenefit of early intervention, long term cost effectivenessof such interventions and the end point of therapyremain unclear. Pharmacotherapy must be used withcaution in children with prediabetes. Prediabetes is acondition defined as having blood glucose levels abovenormal but below the defined threshold of diabetes. Itis considered to be an at risk state, with high chancesof developing diabetes. While, prediabetes is commonlyan asymptomatic condition, there is always presence ofprediabetes before the onset of diabetes.
