• W3149290151 abstract "Частота и тяжесть клинического течения легочных грибковых инфекций в последние годы нарастают быстрыми темпами, что обусловлено несколькими факторами. В повышение частоты этих заболеваний вносит немалый вклад рост числа иммуноскомпроме тированных пациентов со злокачественными и гема тологическими заболеваниями и ВИЧ инфекцией, а также больных, получающих иммуносупрессивные препараты по поводу трансплантации органов или аутоиммунных воспалительных заболеваний. Легоч ные грибковые инфекции стали чаще выявляться в связи с развитием новых диагностических методов, таких как компьютерная томография (КТ) и позит ронно эмиссионная томография (ПЭТ), бронхоско пия, медиастиноскопия, видео ассистированная то ракоскопическая биопсия. В то же время появление новых видов терапии существенно расширило воз можности врача, занимающегося лечением таких состояний. Если раньше противогрибковая терапия была ограничена использованием амфотерицина В, флуцитозина и нескольких производных азола, сегодняшний выбор фармакологических средств включает в себя новые мощные препараты на основе азола с расширенным спектром противогрибковой активности, новые липидные формы амфотерицина В и новый класс противогрибковых средств, полу чивших название эхинокандинов. В связи с измене ниями частоты, выявления и лечения легочных грибковых инфекций Американское торакальное общество (АТS) создало рабочую группу экспертов по грибковым инфекциям для разработки клиничес кого соглашения по современным принципам тера пии грибковых инфекций, встречающихся в пульмо нологической практике и интенсивной терапии. В данном документе выделяются 3 основных аспекта: эндемические микозы (гистоплазмоз, споротрихоз, бластомикоз и кокцидиомикоз); грибковые инфек ции у иммуноскомпрометированных и критически тяжелых больных (критпококкоз, аспергиллез, кан дидоз и пневмоцистная пневмония); редкие и новые грибковые инфекции." @default.
