The disruption of homeostatic mechanisms during starvation or injury has consequences not only for nutritional status but for the regulation of fluid and electrolytes. In this chapter, the principles of water/electrolyte balance and their impact on metabolic processes relevant to clinical nutrition are discussed. The loss of lean and fat body mass, the counter-regulatory hormone response, and the production of inflammatory mediators all have important consequences for the balance of fluid and electrolytes between the extracellular fluid (ECF), intracellular fluid (ICF), and interstitium. The chapter discusses the implications of water and sodium metabolism in nutrition therapy for specific clinical conditions. Future studies aim to evaluate the impact of balanced intravenous solutions and assess the value of individualised therapy and goal-directed fluid therapy (GDFT) in the clinical setting." @default.
