- W3155258382 abstract "Abstract Fibroblast growth factor 23 (FGF23) was discovered as a phosphaturic hormone, playing a key pathophysiological role in renal phosphate-wasting disorders. FGF23 is mainly produced in bone and protects against hyperphosphatemia by downregulating proximal renal tubular abundance of sodium–phosphate cotransporters and of 1α-hydroxylase, leading to increased renal phosphate excretion and reduced 1,25(OH)2 vitamin D (1,25D) hormone production, respectively. During recent years, it has become clear that FGF23 not only acts on proximal renal tubules but also targets the distal nephron, where it activates WNK (with-no-lysine kinase) signaling. FGF23-mediated WNK activation in the distal nephron upregulates tubular reabsorption of calcium and sodium through increased apical membrane abundance and activation of the epithelial calcium channel transient receptor potential vanilloid-5 and of the sodium chloride cotransporter, respectively. Hence, FGF23 not only targets proximal renal tubules as a phosphaturic and vitamin D–regulating hormone but also has additional functions in the distal nephron as a calcium- and sodium-conserving hormone. Given the key importance of the WNK signaling pathway for volume homeostasis and blood pressure control, FGF23-mediated WNK activation in the distal nephron may promote the development of hypertension and of vascular calcification in diseases associated with excessive FGF23 secretion such as chronic kidney disease, when renal disease processes interfere with the ability to hormonally counterbalance the calcium- and sodium-conserving effects of excessive FGF23 signaling." @default.
