- W3155321953 abstract "Introduction: Behcet disease (BD) is rarely accompanied by amyloidosis, and treatment for this condition has not yet been established. We report a BD patient who was diagnosed with intestinal amyloidosis and was successfully treated with tocilizumab (TCZ), a humanized anti-human interleukin-6 receptor antibody. Patient concerns: A 44-year-old woman was admitted to our hospital because of abdominal pain and diarrhea. She experienced periodic remittent fever in her childhood. She was diagnosed with juvenile idiopathic arthritis during that period and was treated with aspirin. She developed aphthous stomatitis, uveitis, and erythema nodosum prior to admission. Diagnosis: Computed tomography findings showed inflammatory lesions in the small and large intestines. Edematous and reddish mucosa was observed in the terminal ileum and ascending colon on colonoscopy, and bleeding and erosive lesions were observed in the small intestine on capsule endoscopy. The patient was diagnosed with amyloid A (AA) amyloidosis based on the histopathological findings and BD based on the criteria for diagnosing BD. Laboratory findings showed high levels of C-reactive protein and serum AA, and the presence of human leukocyte antigen B-51 and serum amyloid A 1.3 and serum amyloid A -13T alleles. Both alleles were risk factors for developing AA amyloidosis in Japanese individuals. Interventions: Infliximab, an anti-tumor necrosis factor-α antibody, was administered, but the patient developed high-grade fever after the administration of this drug. After administering prednisolone as a temporary treatment, TCZ was provided. Outcomes: Following TCZ treatment, abdominal pain and diarrhea immediately improved, and the patient was able to resume oral diet. Computed tomography showed improvements in intestinal inflammation, and the levels of serum AA decreased. Conclusion: Continuous inflammation and genetic background might be risk factors for developing BD with amyloidosis, and TCZ may be a favorable treatment for BD complicated with amyloidosis." @default.
