Although it's relatively inexpensive and causes minimal side effects,4 it should be discontinued when normal weight is restored. A 1998 study by Demling and DeSanti assessed eight patients with large, nonhealing wounds. They found that optimizing nutrition alone for four weeks didn't significantly increase patients' weight or rate of healing; however, when oxandrolone was added at a dose of 10 mg BID, five wounds healed completely and three wounds showed a 75% improvement during the 12-week study period. There was a significant correlation (r = 0.67) between restoration of weight loss and closure of the previously nonhealing wound. In this study, increased healing was most evident when 50% of the lost weight had been restored.4 Therefore, it's important to assess both the wound and nutritional status of the patient in order to rule out wound infection and sepsis.5 Patients' caloric intake and weight should be monitored daily and hematocrit and hemoglobin counts and serum albumin levels checked. The following consultations will also help correct the nutritional deficit by setting the healing process in motion6-8: * a nutritional consultation to recommend oral nutritional supplementation * an occupational therapy consultation to improve fine motor skills and self-feeding capability * a physical therapy consultation for optimal positioning, muscle strengthening, balance, and ambulation * a consultation with a wound ostomy continence nurse to alleviate wound pain and odor and to select specialty bed therapy, if necessary Patients are sometimes discharged from the acute-care facility long before their nutritional deficits are addressed or their wounds are healed.
