Ces éléments guident le choix du soluté selon la situation clinique. Le Ringer lactate est un soluté balancé bien toléré dans la majorité des situations cliniques en anesthésie-réanimation. Cependant, le Ringer lactate est un soluté hypotonique. Dans un contexte de neurochirurgie ou de neuroréanimation, le choix doit se porter vers un soluté balancé isotonique. En réanimation, l’administration de solutés balancés est associée à une amélioration de la survie et une diminution du recours à l’épuration extra-rénale par rapport au NaCl 0,9 %. Il est suggéré d’utiliser en première intention les solutés balancés en anesthésie-réanimation pour l’expansion volémique. Balanced fluid solutions exhibit sodium and chloride contents close to those encountered in human plasma. Balanced fluid solutions have a strong ion difference (SID) closer to the one of plasma (40 mmol.L−1) than NaCl 0.9% whose SID is 0 mmol.L−1. NaCl 0.9% administration is responsible for hyperchloraemic metabolic acidosis. Balanced solution administration has minimal effect on acid-base equilibrium and electrolyte concentration. Balanced fluid solutions vary from one another in the anion that replaces chloride (acetate, malate, gluconate, lactate), their strong ion difference (24 to 50 mmol.L−1) and their tonicity. These characteristics drive fluid choice according to the clinical setting. Ringer Lactate is a balanced fluid solution that is well tolerated in most settings in anaesthesia. However, Ringer Lactate is a hypotonic solution. In a context of neurosurgery or neurocritical care, choosing a balanced isotonic fluid is more appropriate. In intensive care, the administration of balanced fluid solutions is associated with an improvement in survival and a reduction in the need for renal replacement therapy as compared to NaCl 0.9%. It is suggested to use balanced fluid solutions in anaesthesia and intensive care, in the first instance, for restoration of circulating volume." @default.
