• W3163630965 abstract "250 Introduction: Diffuse increased uptake of FDG in renal parenchyma (DRU) is sometimes observed in PET/CT using FDG [Fig. 1]. Several case reports have shown DRU was seen in patients with interstitial nephritis, glomerulonephritis, malignant lymphoma, or IgG4-related disease; however, clinical characteristics associated with DRU have not been fully investigated. Our aim was to investigate clinical characteristics of patients with DRU, especially focusing on renal function. Methods: We defined DRU as bilateral diffuse increased uptake in renal parenchyma compared to liver without hydronephrosis. From January 2011 to March 2015, a total of 10791 FDG-PET/CT scans were performed at our institute, and we found 57 cases (0.53%) in 55 patients with DRU (male:female=24:33, mean age 62 yr. range 0-84 yr). To investigate their renal function, we assessed 42 scans in 42 patients with a serum creatinine level available within two days before or after PET/CT scan, and with more than two serum creatinine levels available within 30 days before or after PET/CT scan. We also included 42 scans with normal renal uptake that were frequency-matched with DRU group for age and gender as a control group. We evaluated four parameters: creatinine level (Cr) and estimated glomerular filtration ratio (eGFR) within two days before and after PET/CT scan; Cr-ratio: Cr divided by the lowest creatinine level within 30 days before and after PET/CT scan (Cr-min); max-Cr-ratio: the highest creatinine level within 30 days before PET/CT scan divided by Cr-min. We compared these parameters between the two groups. Results: Underlying diseases of 57 scans with DRU were malignancy (n=39; carcinoma (n=23), leukemia/lymphoma (n=15), sarcoma (n=1)), collagen disease (n=4), IgG4-related disease (n=2), multicentric Castleman disease (n=1), sarcoidosis (n=1) and others (n=10). 12 of them were performed within 30 days after chemotherapy, and drugs were cisplatin (n=4), fluorouracil (n=3), methotrexate (n=3), cyclophosphamide (n=2), cytarabine (n=2), vincristine (n=2). In renal function analysis, no significant difference was observed in Cr between the DRU group and the control group (median: 0.73 vs. 0.68 [mg/dL], p=0.097), but eGFR was significantly lower in the DRU group than in the control group (median: 68.7 vs. 88.6 [mL/min/1.73m2], p=0.045) [Fig. 2], and more patients showed eGFR < 60 mL/min/1.73m2 in the DRU group than in the control group (40% vs. 17%, p=0.029). No significant difference was observed in the Cr-ratio between the DRU group and the control group (median: 1.21 vs. 1.17, p=0.054), but more patients showed the Cr-ratio ≥ 1.5 mg/dL in the DRU group than in the control group (38% vs. 12%, p=0.011). The max-Cr-ratio was significantly higher in the DRU group (median: 1.59 vs. 1.25, p=0.004) [Fig. 3], and more patients showed the max-Cr-ratio ≥ 1.5 mg/dL in the DRU group (55% vs. 24%, p=0.007). Conclusions: Our data indicate that DRU could indicate acute kidney injury, although patients with DRU were clinically heterogenous." @default.
