Les lésions du CPF (traumatismes crâniens, accidents vasculaires, tumeurs, démences dégénératives, affections psychiatriques) engendrent un état clinique aux multiples facettes retentissant sur les comportements du patient qui peuvent être diminués (apathie) ou mal contrôlés (désinhibition, troubles des conduites sociales) avec de multiples conséquences médicolégales. Une meilleure connaissance du cortex préfrontal et de ses troubles est souhaitable pour mieux comprendre les mécanismes qui sous-tendent les phénomènes d’addiction ou certaines pathologies psychiatriques (dépression, schizophrénie notamment) et pour mieux repérer sa responsabilité dans les difficultés de reprise de vie professionnelle ou sociale après traumatisme crânien ou dans la survenue d’actes délictueux inexpliqués. The prefrontal cortex (PFCx) accounts for 30% of the cortical areas in humans and includes important neural units which encode working memory, conditioning and reward value, inhibition of inappropriate behaviors, error detection, conflict monitoring and cognitive control. This associative cortex works out behaviors in adequacy with social and environmental contingencies and internal needs. PFCx lesions (head injury, stroke, tumors, degenerative dementias, psychiatric disorders) generate a multifaceted clinical picture influencing the patient's behaviors, which may be decreased (apathy) or poorly regulated (disinhibition, social behavior disorders) with multiple medico-legal consequences. A finer analysis of the prefrontal cortex and its disorders is essential to better understand the mechanisms underlying addictive behaviors or some psychiatric disorders (particularly depression and schizophrenia) and to better identify its responsibility in the difficulties of resuming professional activity or social life after a brain trauma or in the occurrence of unexplained criminal acts." @default.
