- W3174647532 abstract "1487 Objectives: The unilateral lung transplantation is one of curative surgical methods for chronic respiratory dysfunction. However, few studies have reported about the course of respiratory function in the residual non-transplanted original lung. We evaluated the serial change of both transplanted lung and non-transplanted original lung function before and after unilateral lung transplantation using scintigraphy and CT volumetric. Methods: We analyzed retrospectively twenty-two lung patients who were undergone unilateral lung transplantation for the fundamental therapy of refractory pulmonary fibrosis. All patients were done both Tc-99m MAA perfusion scintigraphy and Kr-81m lung ventilation scintigraphy before and after lung transplantation. At the same time CT volumetric were done in each patient. After the operation, these examinations were evaluated annually within 2 years. On the both anterior and posterior planar images, the count ratios of non-transplanted lung to the transplanted lung (N/T ratio) were obtained. The CT volumetric study was also performed and the lung volume ratios of the non-transplanted lung to the transplanted lung (N/T volume ratio) were obtained using ZIO STATION (Zio software, Tokyo, Japan). Results: Before the transplantation, the mean values of N/T ratio were 0.89 on the anterior images and 0.91 on the posterior images. On the CT volumetric, the mean values of N/T volume ratio was 0.88. After the transplantation the mean value of N/T ratio was 0.43 and 0.42 after 1 year and the 0.29 and 0.31 after 2 year. The mean values of N/T volume ratio were 0.2 after 1 year and 0.1 after 2 year. After the unilateral lung transplantation, it was confirmed that both ventilation and perfusion remained in the non-transplanted lung despite the remarkable volume loss, but they declined over time. Conclusions: After the unilateral lung transplantation, both perfusion and ventilation were gradually and continuously decreased in non-transplanted lung. Both Tc-99m MAA perfusion scintigraphy and Kr-81m lung ventilation scintigraphy was useful tool for monitoring respiratory function in patients who would be undergone unilateral lung transplantation." @default.
