- W3176292044 abstract "Cerebral cavernous malformations (CCM) and venous anomalies (VA) are among the most commonly encountered intracranial vascular lesions. Any treatment strategy must incorporate a thorough understanding of the natural history and predicted risks of hemorrhage. Due to the widespread use of cranial imaging, many lesions are now incidentally discovered. There are several genetic implications related to CCM with three gene loci implicated in CCM pathogenesis. CCM can also present with hemorrhage or seizures, depending on lesion location. Many CCM can be managed conservatively. Surgical intervention should be considered for accessible lesions causing recurrent symptoms. Surgical approach should account for prevailing white matter tracts to minimize neurologic deficits. The efficacy of stereotactic radiosurgery is debated, though some are in favor of radiating symptomatic lesions in eloquent areas. Radiosurgery outcomes are clearly inferior to microsurgical outcomes, and any consideration for radiosurgery should include possible surgical interventions by an experienced neurosurgeon. Venous anomalies are benign lesions that drain normal brain tissue. Every effort should be exhausted to preserve these structures. Resection of VA can result in serious morbidity from ischemia compromise." @default.
