We were once again called to examine the data showing the inequality of women in academic medicine. The year 2017 marked when female medical students first outnumbered males at 50.7%, 1 and yet the pipeline into academic medicine faculty is not keeping up. In 2018, women only comprised 25% of full professors and 37% of associate professors. 2 I have come to learn over my career that there are many barriers to women achieving academic promotion, including lack of role models and mentorship, bias in research opportunities, work–life balance challenges, and decreased opportunities for speaking engagements and achievement awards. 3 After overcoming these barriers, there is still one last hurdle for women seeking promotion: garnering referee letter writers. Referee letter writers can be difficult to procure for several reasons. They must be at or above the rank of the candidate’s aspired position. They must also be able to attest to the candidate’s reputation—either regionally for associate professor or nationally/internationally for full professor. They do not always need to be someone the candidate knows, though. In fact, many promotion committees require “arm’s-length” letters. These are letters from experts who can provide an objective evaluation of the candidate’s work in the field. One way for a candidate to garner attention for their work is to travel to conferences and present. This step can be much more challenging for women who are likely to attend fewer conferences than their male counterparts. After recently being promoted to full professor, I was inspired to work within my community to help other women seeking referee letters in academic medicine. I reached out to my colleagues in the Alliance for Academic Internal Medicine (AAIM), which represents thousands of internal medicine faculty and administrators at medical schools and teaching hospitals. In both the residency program leaders’ discussion forum and the women leaders’ discussion forum, I asked for willing referee letter writers for women seeking academic promotion. I shared a Google document link with the group, and AAIM featured the opportunity in its weekly email blast AAIM Connection. The Google document has now captured 118 associate and full professors willing to be contacted for letters, including each referee’s area of expertise. This simple referee repository has the potential to create meaningful change in moving the metrics on academic promotion for women. I encourage other specialty organizations to follow suit.
