- W3182024967 abstract "PURPOSE: Endurance athletes (FIT) are known to have lower orthostatic tolerance as compared with non-athletes (UNFIT). Previous studies showed a greater decrease in stroke volume (SV) and cardiac output (CO) during orthostatic stress in FIT as compared with UNFIT. However, it is still unclear how cerebral hemodynamics during orthostasis would be different between FIT and UNFIT. We hypothesized that the reduction in cerebral blood flow (CBF) in response to orthostatic stress with lower body negative pressure (LBNP) would be greater in FIT than in UNFIT. The purpose of this study was to test this hypothesis by comparing the CBF response to severe orthostatic stress with LBNP between FIT and UNFIT. METHODS: Eleven male FIT (21 ± 3 years old, VO2Max: 59 ± 5 ml/kg/min) and 11 male UNFIT (25 ± 7 years old, VO2Max: 36 ± 8 ml/kg/min) underwent orthostatic stress test using graded LBNP. LBNP was increased to -15 mmHg, -30 mmHg, and -40 mmHg every 5 minutes, followed by an additional -10 mmHg increment every 3 minutes until pre-syncope. Heart rate (HR, ECG), mean arterial blood pressure (MAP, Finometer), middle cerebral artery blood flow mean velocity (MCAv, transcranial Doppler), and SV (echocardiography) were assessed during LBNP. CO was calculated by SV × HR. Two-way repeated ANOVA was used to compare them between FIT and UNFIT. Moreover, the slopes in the change of CO and MCAv against LBNP were calculated by linear regression analysis for each subject, and compared by unpaired t-test between FIT and UNFIT. RESULTS: MAP was maintained until presyncope in both groups. CO and MCAv gradually decreased as LBNP increased in both groups (P < 0.001, the effect of LBNP), but there was no difference in the trend between FIT and UNFIT (CO: P = 0.634, MCAv: P = 0.278, the interaction of LBNP×FIT or UNFIT). However, the slope of MCAv against LBNP was larger in FIT than in UNFIT (-0.26 ± 0.12 cm/sec/mmHg vs. -0.14 ± 0.10 cm/sec/mmHg, P = 0.022). The slope of CO against LBNP tended to be larger in FIT than in UNFIT (-0.03 ± 0.02 L/mmHg vs. -0.02 ± 0.01 L/mmHg, P = 0.074). CONCLUSIONS: The magnitude of the decrease in MCAv and CO due to the same orthostatic stress was greater in FIT than in UNFIT. The present finding suggested that the greater CBF decrease during orthostatic stress was caused by the greater CO decrease in FIT as compared with UNFIT." @default.
