- W3186342114 abstract "Unexplained Ventricular Fibrillation (UVF), defined as a resuscitated cardiac arrest without cardiac, respiratory, metabolic, and toxicological etiologies is rare and poorly understood in children and teenagers. The primary objective of the study is to describe the clinical characteristics, current evaluation and outcomes of patients initially diagnosed as UVF under 21 years of age. 12 centers completed online data collection for this retrospective cohort study. Patients ≤ 21 years old with sentinel events between January 1, 2005 to July 31, 2019 were included. A total of 38 patients (76% male, mean age 12 years) were enrolled in the study. Cardiac events occurred at rest in 9 (24%), with activity in 18 (48%), and during sleep in 4 (11%) patients. Automatic external defibrillator was used in 79% (n=30) patients with average of 2-3 shocks and average time to first shock of 7 min. Five patients (13%) had prior history of syncope and 2 (5%) had seizures. Although all patients had an echocardiogram and EKG, the frequency of other cardiac testing ranged from 82% for cardiac MRI to 11% for provocative drug testing. Only 12 patients (32%) underwent EP testing and 23 (62%) underwent genetic testing (no pathologic variants detected; 19 (83%) patients had variants of unknown significance). ICD was implanted in 33(87%) of patients, 24 (63%) during initial hospitalization and 9 (23%) within two years after the cardiac event. At the mean follow up of 44 months (6 months-10 years), there were 22 appropriate and 10 inappropriate shocks. One patient (2%) died during initial hospitalization. Due to young age at presentation, ICD was not implanted in four (11%) patients. Three of them received AEDs. Unexplained Ventricular Fibrillation in young is rare as only 38 patients from 12 centers fulfilled diagnostic criteria. Cardiac events occurred most of the time with exertion. The yield of gene testing is low with available commercial panels. The majority of young patients received an ICD, with two thirds having appropriate shocks. Since cardiology evaluation varies greatly between the centers, there is need for a more uniform approach in evaluation of such patients." @default.
