- W3186471273 abstract "The Apple Watch Irregular Rhythm Notification (IRN) feature analyzes pulse data using photoplethysmography and identifies prolonged episodes of irregular rhythm suggestive of atrial fibrillation (AF). IRN is FDA cleared for those with no previous history of AF. However, clinicians are increasingly using these devices for AF detection and management in those with a known AF history. To determine the accuracy of the IRN feature in subjects with a previous diagnosis of paroxysmal AF. Ten subjects with a history of paroxysmal AF and either an implantable cardiac monitor (ICM) or cardiac implanted electronic device (CIED) with < 1% ventricular pacing were fitted with an Apple Watch Series 5 for 6 months. AF episodes were compared between the ICM/CIED and IRN. The primary endpoints were sensitivity, specificity, PPV, and NPV of the IRN by subject for AF ≥ 1 hour. Secondary endpoints were sensitivity and PPV by AF episode ≥ 1 hour. Analysis was limited to a maximum of 10 ICM/CIED episodes per subject and included only those AF episodes occurring during watch use confirmed by hourly activity data. Mean age was 67.5y ± 7.7y. Three subjects (30%) were female. Implanted devices consisted of 7 ICMs and 3 CIEDs. 5 subjects had AF on ICM/CIED while the watch was worn, of which 4 were detected by IRN. There were no false positive IRN detections by subject (“by subject” 80% sensitivity, 100% specificity, 100% PPV, and 83.3% NPV). 1 subject had AF only when the watch was not worn. There were a total of 34 AF episodes on ICM/CIED, 18 of which occurred while the watch was being worn. Of these, 12 were detected by IRN with 1 false positive (by episode sensitivity = 66.7%, PPV = 92.3%). In a population with known AF, the Apple Watch IRN feature detected 4/5 subjects with AF and no subjects were falsely classified. Overall accuracy for individual AF episodes was lower as they were either missed by the device (6/18 missed) or occurred while the watch was off (16/34). The current IRN algorithm appears accurate for AF screening as currently indicated, but increased sensitivity and wear times may be necessary for disease management.
