Our study aims to identify clinical or surgical factors correlated to the TURB-RC histological concordance in terms of HV detection, usually suboptimal, in order to better manage the malignancy. METHODS: Consecutive patients undergone RC between 2000 and 2019 at a single Institution with pathological evidence of HV were included. Patients with diagnosis of BCa-HV both at RC and at the correspondent previous TURB were enlisted in the TURB-RC Concordance Group (CG) whereas patients with only evidence of BCa-HV at RC in the TURB-RC Non-Concordance Group (NCG). The analyses focused on the identification of factors able to predict the histological concordance between TURB and RC. Demographic data and tumor characteristics were considered as clinical factors. Surgical factors were the source of energy (SoE, mono- vs bipolar), surgeon experience (</≥100), execution of reTURB, number and maximal core length (MCL) of specimens at TURB. RESULTS: Overall, 81 patients were included. The RC was performed for a first BCa diagnosis in 84% of patients. A single TURB was performed in 80.2% of patients (19.8% of reTURB), by an expert surgeon in 55.6%, with a mono- and bipolar SoE in 26% and 74% of cases. The median size of the tumor was 30 mm (IQR 24-47) and it was multifocal in 18.5% of cases. The squamous was the most common HV both at RC and TURB histopathology (50.6% and 33.3% respectively). The TURB histopathology revealed a pT1 in 11%, pT2 in 82.8% and Cis in 6.2% of cases, a number of specimens ≤50 in 42%, 51-199 in 43% and ≥200 in 15% and a MCL ≥10 mm in 42% of cases. At least 1 HV was found at TURB (CG) in 49 patients (60.5%) while no HV was found in 32 patients (39.5%, NCG). Among the 10 patients undergone reTURB in the CG, 6 had HV at TURB, 1 at reTURB and 3 at both. No significant differences between CG and NCG were reported in terms of type of HV at RC (p=0.153), BCa recurrency (p=0.143), performance of reTURB (p=0.919), surgeon experience (p=0.332), SoE (p=0.680), tumor size (p=0.172), BCa multifocality (p=0.132) and pT stage at TURB (p=0.384). Longer specimens (MCL) were found in the CG, both as a continuous variable (12.5 vs. 10 mm, p=0.014) and with a cut-off of 10 mm (p=0.012). At uni- and multivariable analyses, MCL>10 mm represented an independent predictor of concordance [OR 2.95; CI (1.01-8.61); p=0.048]. CONCLUSIONS: Longer specimens at the TURB yield a higher chance to detect HV before RC. In this light, improving the quality of bladder resection means improving the management of the BCa.
